The majority of patients with acute myeloid leukemia (AML) treated with VAD therapy are considered to have no response to treatment and are therefore considered to be in the refractory phase of the disease. A subset of these patients, however, was reported to demonstrate a complete remission after discontinuation of VAD therapy. We, herein, identified a group of patients with AML who were treated with VAD and had a continuous complete remission (CCR) off VAD therapy for >or=12 months. Between 1983 and 1994, 13 patients with AML in CCR were treated with VAD therapy. The median age was 45 (range, 19-72) years, and all but one of the patients (12 of 13) achieved a CCR. Six patients subsequently remained in CCR off VAD therapy for >or=12 months and were referred to us to be evaluated for a possible second allogeneic transplantation. Two of the 6 patients (33%) (2 of 6, 33%) have survived at least 5 years following discontinuation of VAD therapy.
